Full details of what caused the error (i.e: What were you doing before the error occurred?)

Exact text of error message- screen shots are welcome whenever possible

Browser used (e.g.: Google Chrome, Safari or Firefox)

Device type (e.g. iPhone 5c)

Device operating system  (e.g.: iOS 8.2)

Check number the error occurred on

User’s full name and the email address associated with their app account
